Purpose:This study is a randomized, double-blind, placebo-controlled study in which patients who were prescribed esomeprazole 40 mg (NEXIUM) for acute proton pump inhibitor-induced esophageal ulcer (PPI-E), and patients who were prescribed esomeprazole 40 mg or placebo for the same duration of acid suppression (1 or 2 weeks) were randomized to have either placebo or esomeprazole 40 mg. All patients who received the first esomeprazole 40 mg or placebo treatment for 14 days were randomized to receive a placebo for 7 to 14 days. The second esomeprazole 40 mg or placebo treatment for 7 days was followed by a 14-day follow-up period. For the purpose of this study, patients who were prescribed esomeprazole 40 mg or placebo for 14 days were randomized to receive a placebo for 14 days and then the first esomeprazole 40 mg or placebo treatment for 14 days followed by a 14-day follow-up period. In addition, the patients who were prescribed esomeprazole 40 mg for the same duration of acid suppression were randomized to receive a placebo for 7 to 14 days followed by a 14-day follow-up period. The primary outcome of the study was the time to first recurrence of the first episode of PPI-E.
The study was a randomized, double-blind, placebo-controlled study (NCT01550045). The study population consisted of 496 patients in the esomeprazole 40 mg group, 869 patients in the placebo group and 728 patients in the esomeprazole 40 mg group (p = 0.09). The study was a randomized, double-blind, placebo-controlled study. There were no differences between the groups in the baseline characteristics, clinical course, acid suppression, hospitalization, pain scores, length of hospitalization and gastric emptying parameters. The main outcome of the study was the time to first recurrence of the first episode of PPI-E. The time to recurrence was not different in the esomeprazole 40 mg group compared to the placebo group (p = 0.12) and the esomeprazole 40 mg group (p = 0.24) (N = 496). The incidence of recurrence in the first episode was significantly lower in the esomeprazole 40 mg group compared to the placebo group (3.5% vs. 3.3%; p < 0.05). In addition, there were significantly fewer patients who had received at least 1 course of treatment (4.4% vs. 2.4%; p < 0.05) and who had received at least 2 courses of treatment (6.1% vs. 3.4%; p < 0.05) in the esomeprazole 40 mg group compared to the placebo group (2.9% vs. 4.2%; p < 0.05). The incidence of recurrence in the first episode in the esomeprazole 40 mg group was significantly lower than that in the placebo group (0.9% vs. 1.1%; p < 0.05).Results:N = 496, placebo = 12.7%; esomeprazole 40 mg = 6.3%; placebo = 3.3%. There was no difference between the groups in the time to recurrence of PPI-E and the time to first recurrence of the first episode (p = 0.09). In addition, the incidence of recurrence was significantly lower in the esomeprazole 40 mg group compared to the placebo group (3.5% vs. 2.4%; p < 0.05).
Conclusion:The incidence of recurrence in the first episode of PPI-E was significantly lower in the esomeprazole 40 mg group compared to the placebo group (3.5% vs. 3%; p = 0.09).
